Printable handout for support group referrals
This is a handout we created for providers, public health groups, and other folks helping serve this population to easily refer patients to Long COVID support groups by printing it and offering it to them in clinic or in person. This handout can help bridge the gap between in-person appointments and resources & community support offered online, as the QR code is scannable with any phone and takes you right to the support group sign-up page for each group. Click the button below to download a printable PDF to use in clinic.
The digital version can also be distributed, as links are embedded on the title of each organization and in the QR code image on this PDF version. We know it can be intimidating and difficult to navigate finding support, so we hope this makes it easier to connect patients with appropriate support.

This large-scale national monthly ECHO webinar series will offer didactic presentations by subject matter experts, examples of emerging best practices and models of care and a facilitated Q&A.
This program is intended primarily for providers who care for patients with Post-COVID Conditions (PCC) and ME/CFS but is open to all health care professionals and all Long COVID and ME/CFS patient-lived experience experts interested in learning more about the treatment of Long COVID and ME/CFS.
